Information
Guidelines: Sharing & ExchangeMedical/Dental
| 1. |
Information pertaining to child’s minor medical care,
minor dental care, emergency medical care and
counseling shall be shared as soon as possible, but
always within twenty-four hours, with the other
parent.
This shall include full written authorization
from each parent to the other parent to obtain any
information concerning any medical, dental,
counseling or other records pertaining to child.

Parents shall promptly inform the other parent of any
illness, which will require medical attention for
child.
|
School
| 1. |
Information pertaining to child’s school data and school
functions shall be shared with the other parent. This
shall include report cards, progress reports, homework, school
pictures/programs and teacher conferences. |
| 2. |
Parent enrolling child in school shall take the necessary
steps with school authorities to; pertaining to
child: (A) list the other parent as a parent,
(B) authorize the release of school information to
the other parent, (C) ensure that the other parent
receives notices, (D) authorize the release of child
to other parent at prescribed times. |
| 3. |
Both parents shall be
listed with the school authorities, to be contacted in
the event of an emergency. |
Childcare
| 1. |
Information pertaining to childcare provider(s) shall be
shared with the other parent. parent. This shall include the name, address and phone number of
the childcare provider(s). |
| 2. |
Parents shall take the
necessary steps with the childcare provider(s) to;
pertaining to child: (A) list the other parent as a
parent, (B) authorize the release of childcare
information to the other parent, (C)
ensure that the other parent receives notices, (D)
authorize the release of child to the other parent
at prescribed times. |
| 3. |
Both parents shall be
listed with the childcare provider(s), to be
contacted in the event of an emergency. |
Activities
| 1. |
Information
pertaining to child’s extra-curricular
activities shall be shared with the other parent.
This shall include schedules and the names and telephone
numbers of activity leaders.

| 2. |
Either
parent may attend any extra-curricular activity in
which child participates.
|
| 3. |
Either parent may
enter child into extra-curricular activities,
which occur during that parent’s parenting time. |
| 4. |
Neither
parent shall enter child into any extra-curricular
activity which involves the other parent’s
parenting time, personal time or money without
obtaining said consent.
